CORE SERVICES

HEALTH INSURANCE PREMIUM & COST SHARING: Helps pay for premiums, co-payments, and deductibles for insurance.

LOCAL AIDS PHARMACEUTICAL ASSISTANCE: Supplemental means of providing ongoing medication assistance when AIDS Drug Assistance Program has restricted formulary, waiting list, and/or restricted financial eligibility

MEDICAL CASE MANAGEMENT: Assists clients in becoming organized and sets plans for clients to cope with medical issues in order to maintain retention in care.

ORAL HEALTH SERVICES: Offers yearly oral health screenings and cleanings. May also be able to assist with payment of other needed dental services.

OUTPATIENT/AMBULATORY HEALTH SERVICES: Provides diagnostic and therapeutic-related activities directly to an outpatient medical setting.

MENTAL HEALTH: Offers access to Gulf Bend Center for psychological counseling services.

HOPWA (HOUSING SERVICES)

FACILITY-BASED HOUSING ASSISTANCE: Assists households that are homeless or at risk of homelessness as a bridge to permanent housing.

PERMANENT HOUSING PLACEMENT: Payment of application fees, credit checks, utility hookup deposits, and reasonable security deposits in order to get them into permanent housing.

SHORT-TERM RENT, MORTGAGE & UTILITY ASSISTANCE (STRMU): STRMU provides short-term rent, mortgage, and utility payments for households experiencing financial crisis because of their HIV diagnosis or a change in their economic circumstance.

TENANT-BASED RENTAL ASSISTANCE: Rental subsidy used to help households obtain or maintain permanent housing, including assistance for shared housing arrangements in private rental housing market. The gross rent of the proposed unit must be at or below the lower of the rent standard or reasonable rent.

HOUSING CASE MANAGEMENT: The central emphasis of a case manager's work focuses on housing issues, including evaluation of housing options, housing stability, and housing specific goals.

SUPPORTIVE SERVICES

EMERGENCY FINANCIAL ASSISTANCE: Provides limited payments to help clients with an emergent need paying for essential utilities, housing, food, transportation, and medication.

FOOD BANK: Provides gift cards to assist clients in maintaining an adequate supply of food.

MEDICAL TRANSPORTATION: Offers transportation assistance to clients with limited/no transportation.

NON-MEDICAL CASE MANAGEMENT: Guidance in accessing medical, social, community, legal, financial, and other needed services.

REFERRAL FOR HEALTHCARE & SUPPORT SERVICES: Assists clients in locating and applying for additional community services/programs.
